This was intended to be a non-functional change. It wasn't. The commit
message was thus wrong. In addition it broke arm, and merged crypto
related code.

Revert with prejudice.

This revert skips files touched in r316370 since that commit was since
MFCed. This revert also skips files that require $FreeBSD$ property
changes.

Thank you to those who helped me get out of this mess including but not
limited to gonzo, kevans, rgrimes.

- Simplify the description of -H to assume 1:1 threading.
- Drop 'process' from description of 'lwp' field and the corresponding
  XO field name.

Fix long name (used by libxo) for the "tdnam" ps(1) keyword.

Undocument "tdnam" (leaving it as an alias), and rename the column

Fix kvm_getprocs(3) error reporting in ps(1).

Previously it just didn't work at all - kvm_getprocs(3) doesn't update
the &amp;nentries when it returns NULL.  The end result was that ps(1) showed
garbage data instead of reporting kinfo_proc size mismatch.
