Apply the following automated changes to try to eliminate
no-longer-needed sys/cdefs.h includes as well as now-empty
blank lines in a row.

This codepath used uint64_t's in place of pointers in structs and
arrays to allow 32-bit code to use 64-bit version of ioctls.  Now
that we support 32-bit compat natively this is no longer needed.

only supports 4 such pop levels.

If the push level is not restored within the processing of the same
HID item, an invalid memory location may be used for subsequent HID
item processing.

Verify that the push level is always valid when processing HID items.
