Apply the following automated changes to try to eliminate
no-longer-needed sys/cdefs.h includes as well as now-empty
blank lines in a row.

All supported compilers (modern versions of GCC and clang) support
this.

Many places didn't have an #else so would just silently do the wrong
thing.  Ancient versions of icc (the original motivation for this) are
no longer a compiler FreeBSD supports.

arm64 has a similar wrapper.  This permits defining &lt;machine/fpu.h&gt; as
the standard header for fpu_kern_*.
